Licensed Vocational Nurse

Will provide education, support and direction for the clinical team. Must be familiar with hospice regulations. This role will have the ability to grow with an exciting new organization.

Job Responsibilities:

• Provide direct patient care under the supervision of DPCS

• Deliver symptom and pain management assessment and support

• Coordinate DME, Supplies and Medications with MD orders

• CC care as directed

• Day time runner covers visits assigned by leadership

• On-call staff covers after hours support and collaboration with Administrator On-Call

Requirements:

• Active LVN License in CA

• Minimum of one year experience in hospice and palliative in a hospice agency or similar field

• Work as part of a cohesive team to meet each patient's needs

• A strong foundation of acute care experience

• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locations

• Ideal candidates must have excellent communication and interpersonal skills

• Able to utilize EMR and other technology to communicate, chart, and update patient's needs
