Certified Home Health Aide (CHHA)

Will provide education, support and direction for the clinical team. Must be familiar with hospice regulations. This role will have the ability to grow with an exciting new organization.

Job Responsibilities:

• Provide direct patient care under the supervision of RNCM

• Assist with activities of daily living

• Communicate DME and supply needs

• Provides RNCM updates and changes

• Charts visits and communicates consistently

Requirements:

• Active CHHA License

• CPR certified

• Minimum of one year experience in hospice and palliative in a hospice agency or similar field

• Work as part of a cohesive team to meet each patient's needs

• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locations

• Ideal candidates must have excellent communication and interpersonal skills, familiarity with hospice regulations

• Able to utilize EMR and other technology to communicate, chart, and update patient's needs

• Great communication and listening skills

• Seeking to work autonomously, while still working with a team

Schedule

• 8 hour shift

• Monday to Friday

• Day shift
