Spiritual Counselor

Ready to join an exciting new team expanding in Southern CA. Searching for a Spiritual Counselor that will enrich the lives of associates, patients and families.

Will provide education, support and direction for the clinical team. Must be familiar with hospice regulations. This role will have the ability to grow with an exciting new organization.

Job Responsibilities:

• Assess the spiritual needs of patients/families/caregivers to provide support and resources

• Evaluates and plans interventions based on findings

• Liaison between patient/family and resources in the spiritual community

• Creates individualized care plans

• Assists with pre and post-bereavement support

• Works with family to set up advanced directives and services

Requirements:

• Theological Degree

• Experience in Hospice Care is preferred

• Minimum of one year experience in hospice and palliative in a hospice agency or similar field

• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locations

• Ideal candidates must have excellent communication and interpersonal skills

• Familiarity with hospice philosophy

• Able to utilize EMR
